HOME VISITS are the most direct support to the individuals and carer, and his or her family.
The main emphasis is on improving the individual's daily activities of life, and on the practical and
social problems that they face. During the visits, social Mobilisers give guidance and support to the
families on caring for the individual, monitoring of the use of medicines and supports, talking and
listening to family. This includes progress or problem ratings, discussion on potential livelihoods
activities, assessment analysis and developing a business plan together, and to make the care of the
individual as easy as possible. It also includes the family and social context in which the individual lives.
